After enlistment, students are required to submit the following:

  1. Photocopy of UP Form 5.
  2. Letter from the company (in official company letterhead) containing a brief description of what the student will do and the name and contact details of student’s immediate supervisor.

Submit the documents to Ms. Mila Ramos. Make sure that she time-stamps the document. Retain a copy of the document for your records.
